The Home Care Coordinator assists with the coordination of home care services for patients discharged from the hospital. The Home Care Coordinator assures appropriate identification of services in compliance with insurance parameters.
2-3 years of Home Health experience or other healthcare public relations is preferred.
Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
